Late Policy

We try very hard to run on time. If you are running late, please notify us. We reserve the right to reschedule you so that we can keep other people on time.

No Show Policy

Please notify the office of appointment cancellations 24 hours in advance.

Referral Policy

We have a very close relationship with our trusted specialists. We prefer you call us for suggestions for specialist referrals so that we can carefully coordinate your care.

Hospital Policy

In case of a medical emergency, call 911 or go directly to the closet Emergency Room (ER). In the event of hospitalization, we prefer the Methodist Healthcare System for your care.

Refill Policy

In order to assist with your health care, we are requesting that you bring an updated medicine list to all your appointments. This will assist your physician and medical assistant with current information when refilling your medicine. You may download and print a blank medication list by clicking here.

All refill requests should specify your preference for brand name or generic substitution. Please specify the strength, dosage and if a ninety-day prescription is required. Also, please include if you would like your prescription sent to the local pharmacy or to a mail order pharmacy. Always verify the above information with your pharmacy.

Our prescriptions are sent electronically. We do not accept refill requests via fax unless your pharmacy does not accept electronic prescriptions.

Prescription Refills After Hours

We ask that you allow 48 hours for refill requests during regular weekday hours. After hours requests and written prescriptions will take an even longer time frame.
Prescriptions written by other physicians or specialists should be refilled by the original physician who issued that prescription unless expressly allowed by your physician at PMA.

Record Release Policy

We reserve the right to charge for medical records in accordance with the Texas Medical Board guidelines.
